We provide comprehensive case management for individuals and families with acute, multiple, and complex problems.  A case manager is assigned to learn about the situation and to help find solutions and resources. A case may be resolved in a one-time phone call or may involve multiple face-to-face meetings. Situations can include domestic violence, a housing crisis, untreated mental health, critical transportation needs, or other challenging life situations.
